CASE MANAGERS please upload Service Plans complete with the following:

~Client's demographic information

~Insurance type and ID#

~Diagnosis and diagnosis code

~Doctor's Name, address, phone, fax, and NPI

~DON Score

~Description of services required

~Weekly Schedule

~Time and Cost information including units per month

PRIVATE PAY clients are accepted at a mimimum of 4 hours of service per day of service.  Call for rates.

Medicaid Accepted. Veterans welcome.
